今回は、自我機能の低下は、どの病気でもおこる、について解説します。

 

精神疾患は、診断基準が世界標準で作られています。

それは世界、どの地域でも、同じように診断ができ、新しい治療を開発するときに診断基準が統一されていないと臨床試験ができない、ということもあります。

 

 

また統計をとるにも、世界標準の診断基準がないと、正確な数字を割り出すことができません。

 

 

ですので、日本でもDSM-Vという診断基準が使われていることが多いです。

 

しかしDSMで診断された同じうつ病の方がいたとして、同じ状態とは限りません。

 

病態の重さを評価するために、病態水準という尺度でも評価を行います。

 

病態が悪ければ、うつ病でも、妄想や幻覚などの精神病症状を認めることがあります。

 

それを精神病圏と呼びます。

 

不安障害でも、病態水準が精神病圏であれば、かなりの注意が必要です。

 

もしかしたら不安症状を前駆症状として、統合失調症に進行する可能性もあります。

 

病態水準は、生育歴を聴取することで、おおむねあたりをつけていきます。

 

大人であったとしても、生育歴を可能な限りとっていくことになります。

 

基本的信頼関係が形成できているか、どうか、このあたりも大事なポイントとなります。

 

内服の使い方も、病態水準に合わせて、選択を考えていく必要があります。

 

自我機能、自分が自分であると認識し、他者と区別することができる機能、も、病態が悪くなると下がってきます。

自我境界もあいまいになり、漠然とした恐怖を感じるようになります。

 

このあたりのトレーニングを受けていない精神科医は最近はかなり多くなってきたように思います。

 

DSMを使用した横断的診断からの標準治療も重要ですが、人の心とはそんなり論理的なものではない部分もあります。

Decline of Ego Functions Can Occur in Any Illness – Explained by a Child Psychiatrist in Nagoya

Today, I would like to explain how a decline in ego functions can occur in any mental illness.

 

Mental disorders are diagnosed using diagnostic criteria that are standardized worldwide. This standardization allows for consistent diagnosis across different regions of the world and ensures that clinical trials for new treatments can be conducted using unified diagnostic criteria.

 

Furthermore, accurate statistics require these global diagnostic standards, as they provide a common framework for calculating reliable figures.

 

In Japan, the DSM-5 diagnostic criteria are widely used.

 

However, even if two patients are diagnosed with depression based on DSM criteria, their conditions may not be identical.

 

To evaluate the severity of the condition, we also use a scale called the level of pathology to assess the condition.

 

If the pathology is severe, even patients with depression may exhibit psychotic symptoms such as delusions or hallucinations. This is referred to as the psychotic spectrum.

 

In cases of anxiety disorders, if the level of pathology is within the psychotic spectrum, extra caution is required. Anxiety symptoms could be a precursor to developing schizophrenia.

 

The level of pathology is generally assessed by gathering information about the patient’s developmental history.

 

Even in adult patients, it is important to obtain as much of their developmental history as possible.

 

Whether or not the patient has developed a basic level of trust is also a key point.

 

Medication use must be carefully selected based on the level of pathology.

 

Ego functions—the ability to recognize oneself as distinct from others—also decline when the pathology worsens. As a result, the boundaries of the ego become blurred, and the patient may experience vague feelings of fear.

 

It seems that fewer psychiatrists today receive training on this aspect.

 

While standard treatment based on cross-sectional diagnosis using the DSM is essential, the human mind is not always purely logical.
