During my training at Tokai University Hospital, I had the privilege of undergoing weekly supervision with a psychoanalysis expert\u2014a system I now deeply appreciate. At the time, I admittedly found it burdensome. But in retrospect, it was a transformative experience that shaped my ability to think analytically.

Supervision sessions, typically costing tens of thousands of yen per session, were provided at no charge. This process taught me the value of introspection as a critical tool for understanding others. It helped me experience firsthand what it feels like to be analyzed, to confront my past, and to see connections to my present.

Why Self-Reflection Matters
Understanding the Past to Understand the Present
Psychiatry involves exploring a patient\u2019s history to address their current issues. Without self-reflection, it\u2019s hard to know:

What questions to ask.
How to handle the information gathered.
How a person\u2019s past connects to their present mental state.
Simply filling out a questionnaire or collecting life history data mechanically is insufficient. Psychiatric care requires deeper insights, which are only accessible if the practitioner has engaged in their own introspection.

Developing Empathy and Professional Insight
Without self-awareness, understanding the emotional and psychological nuances of another person\u2019s experiences is nearly impossible.

How can a practitioner empathize with a patient\u2019s struggles if they have never reflected on their own?
Misguided or superficial interpretations can result if the practitioner lacks depth in their understanding of human experiences.
Differentiating Random Success from Intentional Care
It\u2019s not uncommon for some patients to improve simply because of their inherent resilience or favorable circumstances. However, if the practitioner is unaware of why or how this happens, they may confuse coincidence with competence.

Intentional, evidence-based care is different from random success, and self-analysis equips practitioners to discern the difference.
Challenges in Modern Psychiatry
In Japan, psychiatric training often omits deep psychoanalytic approaches. Many psychiatrists today operate in ways more akin to general medicine or dermatology, focusing on symptoms and medications rather than deeper psychological exploration. While there\u2019s nothing inherently wrong with this, it does mean:

A lack of training in understanding patients\u2019 pasts and emotional intricacies.
A reliance on psychologists or counselors for therapeutic tasks, without the psychiatrist fully understanding the therapy's framework or impact.
Patients with High Self-Awareness
Some patients arrive at clinics with a level of self-awareness that surpasses that of their practitioner. These individuals:

Have spent years reflecting on their lives and challenges.
Are not looking for superficial solutions but a partner in understanding their deeper struggles.
For such patients, a psychiatrist who lacks self-reflection will have little to offer beyond prescribing medication. This disconnect can be detrimental to the therapeutic relationship.

Self-Analysis as a Rite of Passage for Psychiatrists
I strongly believe that engaging in self-analysis should be a mandatory part of psychiatric training. It equips practitioners with:

The ability to handle complex patient histories with sensitivity and insight.
A deeper understanding of their own biases, triggers, and blind spots.
However, in Japan, few mentors emphasize or facilitate this process. Without this foundational training, the field risks losing its depth and humanity.

For psychiatry to thrive, especially in child and adolescent mental health, we must:
