\u30101\u3011Basic Approach to Pharmacological Treatment in ASD
Autism Spectrum Disorder (ASD) is a neurodevelopmental condition characterized by difficulties in social communication and restricted, repetitive patterns of behavior.

Currently, no disease-modifying drugs exist that directly improve the core symptoms of ASD.

However, pharmacological treatment can be effective in alleviating secondary symptoms\u2014such as anxiety, agitation, sleep disturbances, aggression, and attention deficits\u2014and thus improve quality of life (QOL).

Medication should be considered a supportive measure, to be used alongside behavioral therapy and environmental adjustments.

\u30102\u3011Main Secondary Symptoms and Corresponding Medications
\u2460 Agitation, Aggression, and Self-Stimulatory Behavior
Risperidone (Risperdal)
The only medication in Japan officially approved for treating irritability in children (5 years and older) with ASD.
Effective for tantrums, aggression, and self-injurious behavior.
Side effects: weight gain, extrapyramidal symptoms, sedation, elevated prolactin levels.

Aripiprazole (Abilify)
Approved in the US (but not Japan) for irritability and aggression in children with ASD.
Less risk of weight gain compared to risperidone.
As a partial dopamine D2 agonist, it offers a balanced, non-overly sedative effect.

\u2461 Anxiety, Obsessions, Social Anxiety
Selective Serotonin Reuptake Inhibitors (SSRIs)
Examples: Fluvoxamine, Escitalopram
May help with severe anxiety, obsessive thoughts, panic, and social fear.
However, responses vary widely, and in some children symptoms may worsen (increased agitation). Start at low doses with careful monitoring.

\u2462 Inattention, Hyperactivity (ADHD-like symptoms)
Methylphenidate ER (Concerta)
First-line treatment when ADHD symptoms coexist with ASD.
Can improve focus and reduce impulsivity.
Note: Response in ASD is less predictable than in typical ADHD\u2014monitor closely.

Atomoxetine (Strattera)
A non-stimulant ADHD medication.
May be mildly effective for comorbid ADHD in ASD and also help with anxiety or sleep issues.
However, response in ASD may be weaker.

\u2463 Sleep Disorders
Melatonin \/ Ramelteon (Rozerem)
Circadian rhythm abnormalities are common in ASD, leading to sleep-onset problems.
Melatonin is safe and commonly used in children, often as a first-line therapy.
May also help indirectly stabilize behavior.

\u2464 Emotional Dysregulation
Valproic Acid (Depakene)
Useful when mood swings, irritability, or emotional outbursts are severe.
Considered when bipolar tendencies coexist with autism traits.
Requires monitoring of liver function and blood levels.

Intranasal Oxytocin:
Being studied for its potential to improve social functioning in ASD (clinical trials underway in Japan).

NMDA Receptor Antagonists (e.g., Memantine):
Under investigation for possible benefits in reducing rigidity and improving communication.

Minocycline, Vitamin D, Probiotics:
Targeting immune system, inflammation, and gut-brain axis as alternative treatment pathways.
