自閉症スペクトラム障害（ASD）、ADHDに対する内服治療の考え方について、名古屋の児童精神科医が解説

今回は、自閉症スペクトラム障害（ASD）、ADHDに対する内服治療の考え方について解説します。

 

ASDに関して、中核症状である対人相互性の障害に対する薬は研究中であり、保険適応にはなっていない。

 

以下に現状をまとめる

 

① ASDに対する保険適応のある薬

リスパダール（リスペリドン）：攻撃性・興奮の抑制

エビリファイ（アリピプラゾール）：攻撃性・衝動性の抑制

ストラテラ・コンサータ（ADHD併存時に注意力や多動を改善）

📌 中核症状（社会的コミュニケーションの困難・こだわりの強さ）を直接改善する薬は現時点では存在しない。

 

② ASDの中核症状に対する新薬開発

オキシトシン（OXT）：社会的コミュニケーションの改善が期待

バソプレシン受容体拮抗薬：社会性の向上を目指す研究が進行中

グルタミン酸受容体作動薬：神経伝達の調整をターゲットとした新薬が開発中

腸内フローラ・抗炎症治療：ASDの新たな治療ターゲットとして注目

📌 ASDの中核症状に対する薬の開発は進んでいるが、現時点では実用化されたものはない。

 

リスパダールやアリピプラゾールは易刺激性に対して保険適応になっているが、多くの人にとってこれはあまり効果がないと思います。

 

むしろ副作用で集中力が落ちたり、眠くなったり、だるくなったり、するのでマイナスの方が多いと思います。

 

逆にADHD薬は不注意の合併は多いので、うまく効けば、不注意の改善、つまりは並行作業、同時処理の改善により結果としてコミュニケーション力が向上する可能性すらあります。

 

だから私は使うならADHD薬を選択します。

 

研究中の薬は早く臨床試験で結果を出してほしいです。

薬理作用考えると、オキシトシン（OXT）、バソプレシン受容体拮抗薬、グルタミン酸受容体作動薬は効果がありそうに思います。

 

しかし臨床試験にピュアなASDを入れないと意味がないと思いますが、診断する力がある医師は少ないので、臨床試験に入れている対象群がピュアではいから有意差がでない、ということがおきていそうです。

Approach to Medication Treatment for Autism Spectrum Disorder (ASD) and ADHD: A Child Psychiatrist in Nagoya Explains

Introduction

Hello, this is Dr. Koji Kato, director of Wako Clinic, a child psychiatry clinic in Chikusa Ward, Nagoya.

 

Today, I will discuss the approach to medication treatment for Autism Spectrum Disorder (ASD) and ADHD, including current options and ongoing research in this field.

 

1. Medication Treatment for ASD: Current Status

Currently, no medication is officially approved to directly treat the core symptoms of ASD, such as social communication difficulties and repetitive behaviors. However, some medications can help manage associated symptoms, particularly aggression, irritability, and hyperactivity.

 

① Medications Approved for ASD (Japan)

Risperidone (Risperdal): Reduces aggression and irritability

Aripiprazole (Abilify): Helps control impulsivity and emotional instability

Atomoxetine (Strattera), Methylphenidate (Concerta): Used when ADHD symptoms (inattention, hyperactivity) coexist

📌 There is currently no medication that directly improves ASD’s core symptoms (social communication deficits, rigidity in thinking and behavior).

 

2. Ongoing Research on New Medications for ASD

Although no direct treatments are available yet, several promising experimental drugs are being studied for ASD.

 

Oxytocin (OXT): May enhance social bonding and communication

Vasopressin receptor antagonists: Target social interaction difficulties

Glutamate receptor modulators: Aim to regulate neural transmission abnormalities in ASD

Gut microbiome & anti-inflammatory treatments: Exploring links between gut health and ASD symptoms

📌 While these medications show potential, none have been fully validated in clinical practice.

 

3. Practical Considerations for Medication in ASD

In my experience, Risperidone and Aripiprazole are often ineffective for ASD symptoms in many cases. Instead of significantly improving behavior, they can cause sedation, cognitive dulling, and fatigue, which may be counterproductive.

 

💡 On the other hand, ADHD medications can sometimes improve communication skills in ASD individuals.

 

Why?

 

Many ASD individuals struggle with inattention, making it difficult to process multiple social cues at once.

ADHD medications (such as Concerta or Strattera) can improve attention and working memory, indirectly enhancing their ability to engage in social interactions.

For this reason, when medication is needed, I often prioritize ADHD treatments over antipsychotics for individuals with ASD and inattention symptoms.

 

4. Challenges in ASD Drug Development

Developing effective medications for ASD is difficult due to heterogeneous patient groups in clinical trials.

 

Many clinical studies fail to show significant results because they include mixed populations rather than “pure” ASD cases.

Diagnosing ASD accurately requires specialized expertise, but few physicians are highly trained in this area.

As a result, clinical trials may include individuals with coexisting conditions, diluting potential treatment effects and leading to inconclusive results.

📌 To make real progress, clinical trials need to focus on well-defined ASD populations.

 

5. Conclusion: The Role of Medication in ASD & ADHD

✅ There is no single medication that directly treats ASD’s core symptoms.

✅ Risperidone and Aripiprazole are approved for managing aggression, but their benefits may be limited.

✅ ADHD medications can sometimes improve attention and social communication abilities in ASD individuals.

✅ Experimental drugs like Oxytocin, Vasopressin antagonists, and Glutamate modulators may offer future hope.

✅ Accurate diagnosis and personalized treatment approaches are essential for effective management.
