今回は、ADHD治療薬、コンサータ、ビバンセの比較について解説します。

 

 

ADHD（注意欠如・多動症）の治療において、コンサータ（Concerta）とビバンセ（Vyvanse）はどちらも中心的な薬剤ですが、それぞれの作用機序、持続時間、特徴、副作用、適応の違いがあります。

 

1. 薬の分類と作用機序

・コンサータ（一般名：メチルフェニデート）

 

中枢神経刺激薬で、脳内のドパミンとノルアドレナリンの再取り込みを阻害します。

 

結果として、脳内にこれらの神経伝達物質が多く残るため、注意力・集中力が高まります。

 

 

・ビバンセ（一般名：リスデキサンフェタミン）

 

アンフェタミン系の中枢神経刺激薬。

 

リスデキサンフェタミン自体はプロドラッグで、体内で活性化されてから効果を発揮します。

 

ドパミンとノルアドレナリンの分泌を促進し、シナプス間における神経伝達を強化します。

 

2. 効果発現と持続時間

・コンサータ

 

効果の発現は1時間以内と比較的早く、効果は約12時間持続します。

 

徐放性製剤であり、1日1回の服用で済みます。

 

・ビバンセ

 

効果の発現はやや緩やか（服用後1〜2時間）。

 

持続時間は10〜14時間と長く、日中の活動全体をカバーできるよう設計されています。

 

3. 依存性・乱用リスク

・コンサータ

 

メチルフェニデートは依存リスクがあるため、厳重に管理されており、日本では第1種向精神薬に分類されています。

 

薬の徐放機構により、急激な血中濃度の上昇を防ぎ、乱用リスクをある程度抑えています。

 

・ビバンセ

 

プロドラッグであるため、静脈注射や鼻吸入では効果を発揮しません。

 

これにより、コンサータよりも乱用のリスクが低いとされ、欧米では依存性の少ない選択肢として重視されています。

 

4. 副作用の違い

・コンサータ

 

食欲低下、頭痛、入眠困難、腹痛、動悸などが代表的な副作用。

 

心拍数や血圧上昇も報告されており、心疾患のある方には注意が必要。

 

・ビバンセ

 

食欲不振、不眠、吐き気、興奮、易刺激性など。

 

徐々に薬効が現れるため、急激な副作用が出にくいという特長があります。

 

5. 適応と使い分け

・コンサータ

 

6歳以上のADHDに使用可。

 

効果発現が早く、朝の立ち上がりが重要な児童に向いています。

 

日本では長年使用実績があり、小児にも広く用いられています。

 

・ビバンセ

 

成人のADHD治療で使用されることが多く、2023年に日本でも適応が広がりました。

 

遅発性で穏やかな立ち上がりを好む人、コンサータで副作用が強く出る人に適しています。

 

まとめ

コンサータは即効性と持続性に優れ、朝の集中力が必要な子どもによく使用されます。一方、ビバンセは乱用リスクが低く、作用が滑らかに立ち上がるため、成人や副作用を気にする患者にとって良い選択肢となります。どちらも中枢刺激薬ですが、効果の性質・代謝機構・リスクプロファイルに違いがあるため、患者の年齢・生活スタイル・副作用の出方に応じて適切に使い分ける必要があります。

 

コンサータ、ビバンセ、ともに即効性があり、内服後すぐに効果判定をある程度することができます。

 

ADHDは生来の症状であるため、特に不注意は自分でも分かりにくく、実際に不注意症状が改善された状態を体感しないと、不注意が生活のどの部分に悪影響を与えていたのか気が付かないこともあります。

 

そういう意味では、即効性のある、コンサータ、ビバンセは、効果判定しやすく、治療をする意味があるのかどうかを判断するためには使いやすい薬といえます。

ADHD Medications: Comparing Concerta and Vyvanse

Today, I will explain the differences between two key ADHD medications: Concerta and Vyvanse.

 

1. Classification and Mechanism of Action

Concerta (Generic: Methylphenidate)

A central nervous system stimulant that works by blocking the reuptake of dopamine and norepinephrine in the brain, increasing their availability and enhancing attention and focus.

 

Vyvanse (Generic: Lisdexamfetamine)

An amphetamine-based stimulant. It is a prodrug, meaning it becomes active only after being metabolized in the body. It promotes the release of dopamine and norepinephrine and strengthens neural transmission between brain cells.

 

2. Onset and Duration of Effect

Concerta

 

Onset: Within 1 hour

 

Duration: Approximately 12 hours

 

Once-daily extended-release formulation

 

Vyvanse

 

Onset: Slower (1–2 hours post-dose)

 

Duration: 10–14 hours

 

Designed to cover a full day’s activity with a smooth effect profile

 

3. Abuse Potential

Concerta

 

Methylphenidate is classified as a Schedule I psychotropic substance in Japan due to its potential for abuse.

 

However, its extended-release delivery system reduces the likelihood of misuse.

 

Vyvanse

 

As a prodrug, it does not become active if injected or snorted, greatly reducing abuse potential.

 

In the U.S., it is regarded as a safer option in terms of dependency risk.

 

4. Side Effects

Concerta

 

Common side effects: decreased appetite, headaches, difficulty falling asleep, stomach pain, and palpitations

 

May elevate heart rate and blood pressure, so caution is needed in those with heart conditions

 

Vyvanse

 

Common side effects: loss of appetite, insomnia, nausea, restlessness, irritability

 

Due to its gradual activation, acute side effects are generally less intense

 

5. Indications and Ideal Use Cases

Concerta

 

Approved for use in children aged 6 and older

 

Ideal for children who need early morning focus and fast onset of effect

 

Long history of use in Japan, widely prescribed in pediatric settings

 

Vyvanse

 

Primarily prescribed for adult ADHD, with expanded approval in Japan in 2023

 

Suitable for patients who prefer a gentle onset or who experienced side effects with Concerta

 

Summary

Concerta offers rapid onset and strong daytime coverage, making it ideal for school-age children.

 

Vyvanse provides smooth and prolonged effects with a lower risk of misuse, making it a good choice for adults or patients sensitive to side effects.

 

Although both are central nervous system stimulants, they differ in how they work, how they’re metabolized, and how likely they are to be misused. Therefore, selection should be tailored based on age, lifestyle, and individual side effect profiles.

Both Concerta and Vyvanse offer fast-acting results, making it easier to judge their effectiveness shortly after starting. Since ADHD symptoms—especially inattention—can be hard to recognize by the individual, actually experiencing the difference after starting medication can help patients understand how much these symptoms were affecting daily life.
