今回は、２０歳未満の社交不安障害に対する、薬物療法とTMS治療の有効性について解説します。

 

 

20歳未満の社交不安障害に対する薬物療法とTMS治療の有効性について

社交不安障害（SAD: Social Anxiety Disorder）は、思春期から青年期にかけて発症しやすく、対人関係や学校生活に大きな影響を与えます。薬物療法とTMS治療（経頭蓋磁気刺激治療）は、特に重度の症例や、認知行動療法（CBT）だけでは改善が難しい場合に有効な選択肢となります。

 

1. 20歳未満の社交不安障害に対する薬物療法の有効性

① 選択的セロトニン再取り込み阻害薬（SSRI）

第一選択薬として最も使用される薬剤。社交不安障害の中核症状である「不安・恐怖感・過剰な緊張」を抑える効果があります。

長期的な効果が期待でき、社会生活の適応を促進

不安の軽減に加え、回避行動の改善が期待される

 

📌 代表的な薬剤

効果発現まで2～4週間かかるため、焦らず継続することが重要

いきなり中止すると離脱症状が出るため、医師の指導のもと減薬する

 

② ベンゾジアゼピン系抗不安薬（短期使用のみ）

急性期の強い不安やパニック発作がある場合に、一時的に使用される。依存リスクがあるため、長期使用は避ける。

ロラゼパム（ワイパックス）：即効性があり、短時間で不安を和らげる

アルプラゾラム（ソラナックス）：効果が比較的持続し、社会場面での不安を軽減

依存リスクが高いため慎重に使用

2. 20歳未満の社交不安障害に対するTMS治療の有効性

① TMS治療とは？

TMS（Transcranial Magnetic Stimulation, 経頭蓋磁気刺激療法）は、磁気パルスを用いて脳の特定の領域を刺激し、神経の活動を調整する非侵襲的な治療法です。

主に「うつ病」や「強迫性障害（OCD）」の治療法としてFDA（アメリカ食品医薬品局）や日本の厚生労働省に承認されており、社交不安障害への適応も研究が進んでいます。

 

✅ 社交不安障害では、「右背外側前頭前野（右DLPFC）」の低頻度TMSが有効とされる。

✅ 脳の神経ネットワークを調整し、不安を軽減する働きがある。

✅ 薬物療法の副作用が気になる人や、薬に抵抗がある人に適している。

 

② TMS治療の有効性

📌 研究による効果

 

右DLPFC（背外側前頭前野）への低頻度TMSが、社交不安症状を軽減することが報告されている

脳内のセロトニンやGABA（抑制系神経伝達物質）のバランスを調整し、不安の過活動を抑える

また社交不安障害のいくつかの症状はあるが、診断基準を満たす程度ではない方には、効果は保証できず、副作用の方が目立つ可能性が高いです。

 

ＳＳＲＩ、ベンゾジアゼピンは効果を発揮したとして、やめる時に離脱症状が出る可能性、これにも注意が必要です。

 

ＴＭＳ治療は薬物療法以外の治療選択肢としては重要です。

 

苦手な場面を避ければ、次の予期不安は強くなり、症状は悪循環していきます。

それを断ち切るために薬をうまく使うのは重要ですが、安全な使い方をしていく必要はあります。

Hello, I’m Dr. Koji Kato, a child and adolescent psychiatrist at Wako Clinic in Chikusa Ward, Nagoya (Medical Corporation Eihoukai).

 

Today, I’d like to discuss the effectiveness of medication and TMS (Transcranial Magnetic Stimulation) therapy for social anxiety disorder (SAD) in individuals under the age of 20.

 

Social Anxiety Disorder in Adolescents: Medication & TMS Therapy

Social Anxiety Disorder (SAD) often develops during adolescence and can significantly impact school life and social relationships. While Cognitive Behavioral Therapy (CBT) remains the first-line treatment, medication and TMS therapy can be highly effective in moderate to severe cases, or when CBT alone is insufficient.

 

1. Effectiveness of Medication for SAD in Individuals Under 20

① SSRIs (Selective Serotonin Reuptake Inhibitors) – First-Line Medication

SSRIs are commonly used to treat the core symptoms of SAD such as excessive fear, anxiety, and social avoidance.

 

✅ Clinical Benefits:

 

Well-supported by evidence in pediatric SAD

 

Reduces anxiety and promotes social adaptation

 

Helps decrease avoidance behaviors

 

📌 Common SSRIs:

 

Paroxetine (Paxil): Potent anti-anxiety effect but risk of withdrawal symptoms

 

Escitalopram (Lexapro): Fewer side effects; suitable for youth

 

Fluvoxamine (Luvox): Effective for social anxiety in public situations

2. Effectiveness of TMS Therapy for SAD in Individuals Under 20

① What is TMS?

Transcranial Magnetic Stimulation (TMS) is a non-invasive brain stimulation therapy that uses magnetic pulses to modulate brain activity.

It is FDA- and Japan MHLW-approved for depression and OCD, and research is increasingly supporting its use for SAD.

 

✅ In SAD:

Low-frequency TMS to the right dorsolateral prefrontal cortex (right DLPFC) has been shown to be effective.

It helps regulate overactive fear circuits in the brain.

 

✅ Ideal for:

 

Patients who don’t respond well to medications

 

Those concerned about medication side effects

 

② Clinical Evidence for TMS

Right DLPFC low-frequency TMS reduces social anxiety symptoms

 

Balances serotonin and GABA activity in the brain

Long-lasting effects possible, even after discontinuation

 

📌 TMS Limitations:

 

Not covered by insurance in Japan (private treatment only)

 

Requires frequent sessions (typically 5x/week for 4–6 weeks)

 

Response rates around 50–60%

Additional Notes from Dr. Kato:

As I always emphasize, medications should only be used when the benefits clearly outweigh the risks.

 

If a person has some symptoms of social anxiety but does not meet the full diagnostic criteria, medication may not help and may cause more side effects.

 

Both SSRIs and benzodiazepines can cause withdrawal symptoms, so caution is required when stopping them.

 

TMS offers a safe alternative that does not carry dependence risks.

 

Avoiding anxiety-inducing situations may lead to increased anticipatory anxiety, creating a vicious cycle.

Medications, when used safely, can help break that cycle—but only if used correctly.
