今回は、ADHD治療薬、コンサータとインチュニブの使い分けについて解説します。
ADHD治療薬のうち、コンサータ（メチルフェニデート）とインチュニブ（グアンファシン）は、それぞれ作用機序・効果の現れ方・副作用・向いているタイプが異なるため、症状の特徴や患者さんの年齢・ライフスタイルに合わせて使い分けられます。
以下に、それぞれの薬の特徴と、どのように使い分けるかをわかりやすく解説します。
■ コンサータ（メチルフェニデート）の特徴
コンサータは、ADHD治療で古くから使われてきた中枢刺激薬です。主に注意力の改善・多動の抑制・衝動性の軽減に高い効果を発揮します。
作用機序：脳内のドーパミンやノルアドレナリンの濃度を高めることで、注意・集中・実行機能を強化します。
効果の即効性があり、服用後1〜2時間で効きはじめる。
1日1回の朝の内服で、約12時間効果が持続。
小中高生や、仕事・学業で集中力を求められる成人にも使用されます。
ただし、副作用として食欲低下・入眠困難・不安感・心拍数上昇などが出やすい場合があり、慎重な管理が必要です。
■ インチュニブ（グアンファシン）の特徴
インチュニブは、非刺激性のADHD治療薬で、特に多動性・衝動性の強いタイプの子どもに効果を発揮しやすいとされています。
作用機序：脳の前頭前野にあるα2Aアドレナリン受容体を刺激し、神経回路の過剰な興奮を落ち着かせます。
注意力の改善というよりも、気分の安定・落ち着き・衝動の抑制が得意です。
効果の発現には数日〜1週間以上かかるため、即効性はありません。
夕方〜夜に服用することで、睡眠導入にも効果を発揮することが多く、不眠傾向のあるADHDに向いています。
副作用としては、眠気・血圧の低下・倦怠感などが出ることがありますが、コンサータより穏やかで依存性もありません。
■ どう使い分けるか？
集中力の持続が困難で、勉強や仕事のパフォーマンス向上を重視したい場合は、コンサータが第一選択になりやすい。
多動や衝動性が強く、落ち着きがない・感情の起伏が激しいタイプにはインチュニブが向いている。
コンサータで副作用が強い場合や、不安・不眠が悪化する場合に、インチュニブが代替薬として使われることもあります。
両剤を併用するケースもあり、日中はコンサータ、夜はインチュニブというように、時間帯ごとに役割を分けて補完し合うこともあります。
■ まとめ
コンサータは、即効性と集中力改善に強く、日中のパフォーマンスを支える薬。
インチュニブは、多動や衝動を落ち着かせ、睡眠にも良い影響を与える非刺激薬。
患者さんの年齢、生活リズム、症状の出方、副作用への耐性に応じて、医師が丁寧に使い分けを行います。どちらか一方で難しい場合でも、調整や併用により最適な治療バランスが得られることが多いです。
内服は使う限りは、コンサータ、インチュニブに限らず、メリットがデメリットを上回る必要があります。
ADHD薬は精神科薬の中では、効果でやすく、副作用もそれほどつよくないです。
不注意症状は一度改善した状態を体感しないと、不注意が日常生活の何に影響しているのかもわかりにくいです。

■ Features of Concerta (Methylphenidate)
Concerta is a stimulant and one of the most widely used medications for ADHD.
It’s especially effective in improving attention, reducing hyperactivity, and controlling impulsivity.
Mechanism: Increases dopamine and norepinephrine in the brain, enhancing executive functions such as focus and working memory.
Quick-acting: Effects begin within 1–2 hours of taking the tablet.
Once-daily dosing: Provides sustained effect for about 12 hours.
Suitable for school-age children, teens, and adults who require daytime focus for academic or work performance.
Common side effects: Decreased appetite, insomnia, anxiety, elevated heart rate. These require monitoring.
■ Features of Intuniv (Guanfacine)
Intuniv is a non-stimulant medication for ADHD.
It is particularly effective in managing hyperactivity, impulsivity, and emotional dysregulation, especially in children.
Mechanism: Stimulates alpha-2A adrenergic receptors in the prefrontal cortex, calming overactive neural pathways.
More effective in promoting emotional stability, calmness, and reducing impulsivity than in improving attention.
Slower onset: Takes several days to over a week to show effects.
Often taken in the evening, with beneficial effects on sleep — good for ADHD with insomnia.
Side effects: Drowsiness, low blood pressure, fatigue. Generally milder than Concerta, with no risk of dependence.
■ When to Use Which?
If concentration and daytime performance are the priority → Concerta is usually the first-line choice.
If the main concerns are hyperactivity, emotional outbursts, or impulsive behaviors → Intuniv is more suitable.
When Concerta causes side effects like anxiety or insomnia, Intuniv may be considered as an alternative.
In some cases, both drugs are combined: for example, Concerta during the day and Intuniv in the evening to balance focus and relaxation.
■ Summary
Concerta is a fast-acting stimulant that helps improve focus and productivity during the day.
Intuniv is a non-stimulant that calms hyperactivity and impulsivity, and may improve sleep quality.
The choice depends on the patient’s age, symptoms, lifestyle, and side effect tolerance. Even if one medication is insufficient alone, adjusting the dosage or combining both may lead to optimal outcomes.
It’s important that the benefits of any medication outweigh its risks — this applies to both Concerta and Intuniv.
Compared to other psychiatric medications, ADHD medications tend to be more effective with relatively mild side effects.
