今回は、ADHD治療薬の使い分けについて解説します。
ADHD（注意欠如・多動症）の治療に使われる薬には、大きく分けて中枢神経刺激薬と非刺激薬の2種類があります。それぞれの薬には作用の仕方・効果のスピード・副作用・適応するタイプに違いがあり、患者さんの年齢や症状、生活環境に応じて使い分けがされています。
以下に代表的な薬剤とその使い分け方について、表を使わずにわかりやすく解説します。
① コンサータ（メチルフェニデート徐放錠）
特徴：
中枢神経刺激薬に分類され、脳内のドーパミンとノルアドレナリンの濃度を上げて注意力や集中力を高める作用があります。
持続時間はおよそ12時間前後とされ、朝1回の服用で日中の活動をカバーできるのが大きなメリット。
どんな人に向いている？
朝から夕方まで学業や仕事に集中したい子どもや大人。
特に学業上の集中困難やケアレスミスが多いタイプのADHDに効果が出やすい。
ADHD診断が確定し、ある程度生活リズムが安定している方に適しています。
② ビバンセ（リスデキサンフェタミン）
特徴：
こちらも中枢神経刺激薬で、**活性化されるまで時間がかかる「プロドラッグ型」**の薬。
効果が穏やかに立ち上がり、持続時間は12〜14時間程度。
どんな人に向いている？
コンサータで副作用が強かった方や、ゆるやかな立ち上がりが求められる生活スタイルの方。
不注意だけでなく衝動性や多動性も強い方に使われることが多いです。
食欲低下などの副作用がマイルドな場合が多く、感情の起伏が少ない子どもにも適しています。
③ インチュニブ（グアンファシン徐放錠）
特徴：
非刺激薬で、脳の前頭前野の神経伝達を調整することで注意力を安定化させるタイプの薬。
効果はマイルドですが、眠気を伴うことが多く、過活動の抑制やイライラ・興奮を和らげるのに有効です。
どんな人に向いている？
不眠があるADHDの方や、刺激薬に合わなかった方。
特に、夜間の落ち着きのなさ・寝つきの悪さ・情緒不安定なタイプに使いやすい。
多動・衝動性が強い子どもに対して落ち着きを与える目的で使われることも多いです。
④ ストラテラ（アトモキセチン）
特徴：
こちらも非刺激薬で、ノルアドレナリンの再取り込みを阻害して、前頭前野の神経活動を安定させる薬。
ADHDの中核症状（不注意・衝動性・多動）すべてに作用します。
どんな人に向いている？
コンサータやビバンセのような刺激薬を避けたい方（例えば、不安障害を併発している人）。
ゆっくりと効果が現れるため、じっくり治療を続けられる方。
小学生〜成人まで幅広く使われています。

即効性や日中の集中力改善が必要な方には、コンサータやビバンセなどの刺激薬が適しています。
刺激薬で副作用が強い場合や、不眠・情緒不安定が目立つタイプには、インチュニブやストラテラなどの非刺激薬が有効。
内服はADHD治療薬に限らず、使いこなすことが重要です。
メリットがデメリットをうわまわないと使う意味がないです。
比較的、ADHD治療薬は精神科領域の中では使いやすいですが、困っている症状や状況によって使う内服は変わってきます。
容量も中途半端に使うとあまり効果でないこともあります。

② Vyvanse (Lisdexamfetamine)
Category: Stimulant (prodrug)
Mechanism: Slowly activated in the body, offering a more gradual onset.
Duration: Lasts 12–14 hours.
Best for:
Those who experienced strong side effects with Concerta.
Patients who require smoother onset and longer duration.
Individuals with combined-type ADHD (inattention + hyperactivity/impulsivity).
Often better tolerated in children with emotional sensitivity.
③ Intuniv (Extended-release Guanfacine)
Category: Non-stimulant
Mechanism: Regulates activity in the prefrontal cortex to help with emotional regulation and behavioral control.
Key feature: Can cause drowsiness, which helps with hyperactivity and sleep issues.
Best for:
Patients with trouble falling asleep or emotional instability.
Those who cannot tolerate stimulants.
Children with excessive hyperactivity and impulsivity.
④ Strattera (Atomoxetine)
Category: Non-stimulant
Mechanism: Inhibits the reuptake of norepinephrine to improve attention and self-control.
Scope: Effective for all core ADHD symptoms (inattention, hyperactivity, impulsivity).
Note: Takes time to show effects, often a few weeks.
Best for:
Patients who prefer to avoid stimulants, such as those with co-occurring anxiety.
Individuals who can commit to long-term, consistent treatment.
Widely used in both children and adults.

For rapid symptom control or daytime focus: Stimulants like Concerta or Vyvanse are preferred.
For emotional instability, insomnia, or stimulant intolerance: Non-stimulants like Intuniv or Strattera may be more appropriate.
Ultimately, the key is to use these medications correctly and thoughtfully. The benefits must clearly outweigh the risks. Dosage is also important—too low and the medication may not be effective.
ADHD medications, when used properly, are among the more manageable treatments in psychiatry. But the choice of medication should be based on a clear understanding of the symptoms and context.
